How to create a task for a referral

This is only for EMIS practice that require a task for referrals (please refer to SOP)

1. You can create a referral task either within a consultation or using shortcut keys when a patient is retrieved.


Ensure the reason for referral is clearly documented in your consultation notes for the referrals team.

2. Click the drop-down button.

3. Select Referral.

4. The “To” field will automatically populate for the Bookings Team.
You may add a short note such as: “Refer to [service] – see consultation notes.”

For audit and governance, the referral reason should be documented in the consultation entry, not in the task notes.
Some referrals may also require completion of a template, which can be accessed via the Template Picker if needed.

5. If the referral is urgent (e.g. 2WW), click Urgent.
For same-day urgent referrals, speak to the duty GP.

6. Once all details are completed, click Send.
