How to send a referral form (SystmOne) - Abington

This guide shows how to generate, complete and send electronic referral forms (example: dermatology).

Referral Initiation

1. Find the referrals here (little orange arrow)

2. Find the appropriate referral form — if no form exists, please follow the Written Referrals Process. In this example we will complete a dermatology referral.

3. Click "Write Now" to generate the letter.

Form completion

⚠️ If this is your first time creating a form, it will ask you to sign into Microsoft. Use your NHS email and password to proceed.

💡 Make sure you include as much appropriate clinical detail as possible to avoid rejected referrals.

4. How to edit the form: Double-click the checkbox. On the menu that appears, change "Default value" to "Checked" then click "Ok".

5. How to enter text into the form: Double-click the grey text box. Once the menu opens, enter the text you want into the "Default text" box, then click "Ok".

6. Once finished with your form, Save for Future Editing.

💡 This is in case the secretaries need to make amendments before they process your referral.

Sending the referral

7. At this stage, you can send the task immediately to Secretaries to send the referral to the appropriate department.

8. Complete the task — for referrals, select Secretaries from the Team Selection list and click OK.

9. Click "Yes" to confirm the referral.

💡 If you can't find the referral form, write a written referral and send it to the Secretaries.

⚠️ If the form was available but you submitted a written referral instead, Secretaries will either complete the form on your behalf (if enough information is provided) or follow up with you for any missing details.
